Minnesota Aging Pathways Resource Coordinator
Dancing Sky Area Agency on Aging is seeking a 32-hour-a-week FTE Minnesota Aging Pathways Resource Coordinator. This position will be responsible for planning intensive care coordination for people wanting to remain in their home or relocate to the community after a stay in a care facility. Minimum of a bachelor’s degree from an accredited college or university in Nursing, Social Work, Gerontology, or a related human services field. Experience working in person-centered discharge planning, charting, care coordination, or related tasks. Proficient in problem-solving skills, adept at phone etiquette, strong in computer skills, and have an independent work style. Applicants who are within 40 miles of Warren will be required to work in the Warren office a minimum of 3 days a week. If outside a 40-mile radius, work from home status will be granted. Regular travel is required, including throughout the 21-county service area.

Salary Range $54,233 -$65,000 plus benefits. Program Specialist 2, Grade 4.
Hiring Salary $62,000 to $64,000.

Minnesota Aging Pathways Resource Coordinator
Dancing Sky Area Agency on Aging is seeking a full-time, Minnesota Aging Pathways, formally the Senior LinkAge® Line Resource Specialist position. Specialists will provide long-term care options counseling by phone to help consumers understand service options and implement community resources that promote choice and independence. We utilize a person-centered model to link people to services that will help them remain in their homes and promote healthy aging.
Minimum of a bachelor’s degree in social work, nursing, or a related degree. Or a minimum of 4 years’ experience in human services. Proficient in problem-solving skills, adept at phone etiquette, and strong in computer skills. Position open until filled.
Salary Range $56,208-$73,291 plus benefits. Grade 4 Program Specialist 2.
Hiring Salary is $60,000 to $62,000.
Applicants within 40 miles of Warren will be expected to work in Warren at least 3 days a week. Applicants outside the 40-mile radius will be allowed work from home status.
