MN Aging Pathways
R E S O U R C E S P E C I A L I S T
Dancing Sky Area Agency on Aging is seeking a full-time, Minnesota Aging Pathways, formally the Senior LinkAge® Line Resource Specialist position. Specialists will provide long-term care options counseling by phone to help consumers understand service options and implement community resources that promote choice and independence. We utilize a person-centered model to link people to services that will help them remain in their homes and promote healthy aging.
Minimum of a bachelor’s degree in social work, nursing, or a related degree. Or a minimum of 4 years’ experience in human services. Proficient in problem-solving skills, adept at phone etiquette, and strong in computer skills.
Applicants within 40 miles of Warren will be expected to work in Warren at least 3 days a week. Applicants outside the 40-mile radius will be allowed work from home status.
Salary Range:
$56,208 – $73,291 + Benefits
Grade 4 Program Specialist 2
Hiring Salary: $60,000 – $62,000
