Community Living Specialist

 

Dancing Sky Area Agency on Aging provides services, support, and information to help rural older adults thrive. We are a group of dedicated professionals that work to create new services, supports, and relationships to meet changing needs of older adults.   • Proficient problem-solving skills with ability to navigate complex situation with multiple variables.

 

What will you be doing?

  • Build relationships with nursing home staff, community providers and families.

  • Ensure client confidentiality and data privacy.

  • Utilize technology to complete assessment, build support plans and track data to meet state and federal requirements.

  • Research home and community-based services and supports throughout a 21-county service area.

  • Provide ongoing phone follow up support to clients and their families.

  • Meet with individuals to complete assessments, offer options counseling and supportive person-centered planning.

 

What skills are needed?

  • Bachelor’s degree in social work, nursing, gerontology or closely related health and human services degree.

  • Minimum of two years of case management or care coordination in health care setting.

  • Strong time management and organizational skills.

  • Valid driver’s license.

  • Takes initiative and operates with sound judgment.

  • Ability to have a home office preferably in Becker, Ottertail, Hubbard, or Mahnomen counties.

  • Travel within the 21-county services area as needed.

  • Salary Range 49,170-57,036 plus benefits.

  • Target area Becker, Ottertail, and Mahnomen counties
