Community Living Specialist
Dancing Sky Area Agency on Aging provides services, support, and information to help rural older adults thrive. What will you be doing?

Build relationships with nursing home staff, community providers and families.
-
Ensure client confidentiality and data privacy.
-
Utilize technology to complete assessment, build support plans and track data to meet state and federal requirements.

Research home and community-based services and supports throughout a 21-county service area.

Provide ongoing phone follow up support to clients and their families.

Meet with individuals to complete assessments, offer options counseling and supportive person-centered planning.
What skills are needed?

Bachelor’s degree in social work, nursing, gerontology or closely related health and human services degree.

Minimum of two years of case management or care coordination in health care setting.

Strong time management and organizational skills.

Valid driver’s license.
-
Takes initiative and operates with sound judgment.

Ability to have a home office preferably in Becker, Ottertail, Hubbard, or Mahnomen counties.

Travel within the 21-county services area as needed.

Salary Range 49,170-57,036 plus benefits.

Target area Becker, Ottertail, and Mahnomen counties
