Dancing Sky Area Agency on Aging provides services, support, and information to help rural older adults thrive. We are a group of dedicated professionals that work to create new services, supports, and relationships to meet changing needs of older adults.   • What will you be doing?

    • Providing Person centered phone support to Minnesotans including advocacy support and various other topics.

    • Lean Medicare and complete Core Body of Knowledge.

    • Researching community-based supports and services.

    • Develop working relationships with community partners through outreach and training.

    • Develop and maintain professional working relationships with several stakeholders in long term care support services.

    • Identify and assist in completing applications and forms for various benefits someone might qualify for.

    • Attending on going learning and training opportunities.

What skills are needed?

  • Bachelor’s degree in social work, nursing, gerontology or closely related health and human services degree.

  • Minimum of two years of case management or care coordination in health care setting.

  • Excellent customer service skill and interpersonal skills.

  • Strong time management and organizational skills.

  • Valid driver’s license, proof of insurance and criminal background check.

  • Strong understanding of the aging process and caregiver challenges.

  • Demonstrated skills of navigating the long-term care service system.

  • Takes initiative and operates with sound judgment.

  • Salary Range 49,170-57,036 plus benefits.
