What will you be doing?
-
Providing Person centered phone support to Minnesotans including advocacy support and various other topics.
-
Lean Medicare and complete Core Body of Knowledge.

Researching community-based supports and services.
-
Develop working relationships with community partners through outreach and training.
-
Develop and maintain professional working relationships with several stakeholders in long term care support services.

Identify and assist in completing applications and forms for various benefits someone might qualify for.

Attending on going learning and training opportunities.

What skills are needed?
-
Bachelor’s degree in social work, nursing, gerontology or closely related health and human services degree.

Minimum of two years of case management or care coordination in health care setting.
-
Excellent customer service skill and interpersonal skills.
-
Strong time management and organizational skills.
-
Valid driver’s license, proof of insurance and criminal background check.

Strong understanding of the aging process and caregiver challenges.

Demonstrated skills of navigating the long-term care service system.

Takes initiative and operates with sound judgment.
-
Salary Range 49,170-57,036 plus benefits.
